If you have completed volunteer hours outside of an NHS sponsored event that you would like to use for NHS, you must fill out this form. Please return the completed form to Mrs. Carroll BEFORE hours are due.

School Insurance

When a member of NHS you must have school insurance. It is very inexpensive. Must be completed and proof of purchase must be turned in along with application.
